To help make scheduling an appointment more convenient, ENT Associates is pleased to provide these two online options based on your patient status: NEW PATIENT or RETURNING PATIENT. Please read the information thoroughly before clicking to start the process.

NEW PATIENTS – Patients making their first appointment ever with our practice, or patients whose last appointment with us was over three (3) years ago.

Click the button below to start. When the new page opens, click “NEW PATIENT” and complete the electronic “Appointment request” form provided. This will send an email alert to our offices. If you provide an email, our system will generate an autoreply verifying that your correspondence was received and is being processed.

An ENT Associates operator will call you as soon as possible to navigate the scheduling process with you. Please be advised, if your email arrives near the close of business or outside of our business hours, we will respond to you promptly on the next business day.

BEFORE YOU GET STARTED:

• Insurance information will be requested as part of the onboarding process– check  here  for the list of insurance plans that ENT Associates currently DOES NOT ACCEPT.
• If you have already left a voicemail requesting a call back, please do not submit an online request here as it will create duplicate work for our staff and take longer for us to respond.
• Our staff can only contact you by phone. Corresponding by email is NOT an option.
• Please make every attempt to answer when we call, and that your voicemail is set /clear to accept messages.

RETURNING PATIENTS – Patients already established with the practice. NEW!!

Click the button below to start. When the new window opens, click “RETURNING PATIENT” to start a process that gives you direct access to our physicians’ schedules in real time! Use the platform menus to search for and book appointments based on your preferences.
Due to system limitations, these appointment categories ARE NOT self-schedulable. Please contact our office.

BEFORE YOU GET STARTED:

• To maintain the continuity of your care, you MUST schedule with the provider you saw previously.  If special circumstances need to be discussed, please contact our office.
• When prompted for “patient name” you will need to provide it EXACTLY as we have it on file. Any variation or misspelling (e.g., Christopher vs. Chris) will prevent the system from finding you.
• The system is set to look for appointment slots beginning a minimum of seven (7) days out from today. This buffer period allows our staff time to secure and process the referral paperwork often required by insurance. If you want / need to explore more immediate appointment options, please feel free to call our scheduling department.
• If your appointment search initially presents zero (0) time slot options, it means that the physician is booked out beyond the date visible on the screen. Move the calendar forward until the “first available” appointment slots appear.
• Please only schedule ONE (1) appointment. Scheduling multiple appointments will lead to cancellation of all appointments that were self-scheduled.
• If you need to change your original submission, please call our offices for assistance.
